Parents Media Consent Form

This form is to be completed by either:

· the parent
· the legal guardian

whose child is being asked to participate in the production of video, audio or photographic work for The Rockhaq Community/Rockhaq.

I consent to the capture of;

· still images (photographs)
· moving images (film/video)
· audio recordings

I accept that the subsequent use may be in a number of media, including but not limited to;

marketing and educational material, print including newspapers, digital and electronic, including use on social media (for example Twitter or Facebook), and
will be published by Rockhaq, partners and/or agents authorised by Rockhaq.

I understand that:

· editing may be required by Rockhaq, partners and/or agents authorised by Rockhaq for quality or technical purposes
· edited sections may be used in other Rockhaq materials for purposes stated above
· the image(s) and/or recording(s) will be stored securely in appropriate file formats on servers belonging to Rockhaq, partners and/or its authorised agents.

Rockhaq is committed to processing information in
accordance with the GDPR and Data Protection Act 1998. The personal data collected on this form will be held securely and will only be used for administrative
purposes.
